Working in Partnership with Case Managers
We have extensive experience working alongside case managers to implement and stabilise complex support packages. From the outset, we take time to understand the care plan, risk profile and long‑term objectives for each client.
Our partnership approach includes:
- Supporting the mobilisation of new care packages or the transition from existing providers
- Recruiting, training and managing bespoke support teams
- Providing experienced leadership and oversight for complex or high‑risk packages
- Regular communication, reviews and outcome reporting to support ongoing case management
